Home Health Care Services

All care is provided by experienced, qualified and caring staff. Our goal is to assist our clients to live independently in the comfort of their own home.

Case Management
A nurse is assigned to each client to ensure the continuity of overall care planning for all services, assistance in accessing community services, physician communication and care coordination to meet clients needs.

Nursing Service
Licensed nurses provide initial assessments, care planning, patient education and teaching, as well as medical care under the general direction of their physician. Nurses are available on an hourly basis (from 4 hours to 24 hours/day) or on a "per visit" basis.

Rehabilitation Therapists
Physical Therapists (PTs) and Occupational Therapists (OTs) assist you to regain and maintain your independence after an illness or accident. Our therapists provide evaluations and home exercise programs concentrating on activities most beneficial to your recovery and independence.

Home Health Aide Care
Experienced home care aides assists with bathing, grooming and dressing (under the supervision of a nurse) as necessary to facilitate your recovery.

Social Services
Medical Social Workers help patients and families cope with emotional and financial concerns regarding care management or long-term care, assisting with coordination of community resource information for their needs as well as counseling.

Speech/Language Pathologist
Speech Therapists (SLPs) provide treatments to improve speech, swallowing, communication and problem-solving skills, which when combined with other therapies, can significantly contribute to stabilization and recovery.

Nutritional Services
A Registered Dietitian helps patients comply with special diets through diet analysis and individual meal planning around nutritional needs & desires.

Medical Supplies
Assistance in obtaining disposable medical supplies needed during your home care can be arranged by a MultiCare Home Health staff member.

Other
MultiCare Nurses and employees adhere to a strict code of ethics and confidentiality in all aspects of home care and business practices. All services of MultiCare Home Health Services are provided by skilled, caring, experienced staff. Employees are trained in all aspects of home care and the special needs of each patient we care for. Care is available according to individual needs: home visits, hourly shifts, travel assistance and private duty nursing care 7 days a week, 24 hours a day. The initial home care needs assessment is available at not charge. Patients, families, physicians and caring friends can call, write or email us for more information.
